Kevin it looks like it's a early 034 front tensioner case, that had the side tensioner added.

The cover you need is a 2 piece one
1125 021 1200
1125 021 1100

The metal plate is
1128 664 1002

Thank you so much for sharing this. I was at a  state

 I should interject like I told Kevin that saw was constructed from parts and pieces .I had or have no  idea what is actually what .All I know is it came from a big pile of stuff in a box .

The tree trimmer that gave me that thing literally had the bed of a Ford Ranger full of stuff .Saws ,hedge trimmers ,weed wackers and parts parts parts ---and junk .

I actually got most of the saws running including a Mac 700 which is the best parts off of three duds .I looked and looked but try as I might I never found an 066 .
