Something is making the chain or the file move around.
You should be able to see a clear corner like pictured in Kevins and my posts.
I thought they may have provided an incorrect file, but your pictures show the right one.

I dont use any guide, but I second 660's suggestion, try to keep the chain steady.
Keeping the chain rock steady and the file lazer straight as it moves forward is imperative to a good square cutter
